Hey everyone, back again for an exercise and mobility tip for the week.
For today's exercise we will be covering the Side lying rotation also known as the open book stretch as shown in the illustration provided.

To begin, start by lying on your side. Next bring your knees up to a 90 degree angle making sure your hips and knees are stacked on top of each other, as well as your hands. While lying on your side with your arms out-stretched in front of your body, slowly twist your upper body to the side and rotated your spine. Your arms and head should also be rotating along with the spine as shown. Follow your hand with your eyes. Make sure your knees and hips are stacked on top of each other as you rotate. This exercise is a great way to work on thoracic mobility and can be done anywhere you can lay down, making this exercise great before you get up out of bed!

Joanna was born and raised in Southern California. She went to the University of California, Irvine for her undergraduate studies in Public Health Sciences. At UCI, she participated in women's club water polo. She received her Doctorate of Physical Therapy at the University of St. Augustine in San Marcos, CA. She developed an emphasis on manual therapy based on the Dr. Stanley Paris approach. Her clinical affiliations involved a wide range of experiences from inpatient rehabilitation, inpatient acute for post-operative conditions, and outpatient orthopedics.

As a clinician you must not only look locally but globally at how your athletes are moving. In these videos (with just knowing her limited ankle mobility) you can see her before test where she had reproduced her back pain during the squat and no pain while performing the squat after the adjustment.

A little bit of fun with barbells in the landmine position. Reverse lunge followed by overhead squat. Stick with one side for 10 ish reps before switching sides for the reverse lunge. Can add some weight on the top end of the barbell for more of a challenge.

Reading this book when pregnant with my first son is when I Iearned all women in France receive pelvic floor therapy after childbirth. They are automatically sent AND it’s paid for by the government. Yup, so every mother regardless of income has access to it. ✨ In the US, we get one ten minute doctor appointment 6 weeks after our birth and that’s it. The doctor doesn’t check your pelvic floor muscles for strength, your scar for restriction, your abdominals for diastasis recti. We go through one of the biggest physical transformations of our lives and then get a thumbs up like “You’re good. All clear. Carry on.” ✨ The postpartum care in the US leaves a lot to be desired. So don’t wait for a doctor to send you to PT. In most states you can walk right in to see a PT without a doctors referral.✨ If you have - pain with sex
- pee your pants
- pooping problem
- feels like something is falling out
- scar tissue from vaginal or cesarean birth
- abdominal separation
- want to prepare for childbirth
- returning to exercise postpartum... WE CAN HELP. ✨We HAVE to take care of our own bodies and our own vaginas, because no one else is going to, unless you live in France 🇫🇷

☕️Coffee is my favorite thing on this Earth. None of that fancy Starbucks stuff, just some solid, quality black coffee. Surprisingly, I’m not a coffee snob and know a bunch of crazy facts, but it’s what gets me through my days.

👨🏽💻In PT school I easily drank 5 cups of coffee a day (16 oz cups). It’s down to about 2-3 now.
.
If I had to choose, I’d pick coffee over breakfast every morning. And i loooove food. 🙈
.
✔️Start my day off with positive thoughts 💭 of going Tibia Good Day. It’s crazy how the powerful the mind is. My first week of my contract they overloaded me with evals all day every day and everyone kept making negative comments like “that sucks”, “I hope you survive”, “ahhh I remember when I started and felt your pain”, etc...
.
➕I chose to be positive and was grateful for a job and to make more money than the average US salary (despite my insanely high debt) and have food everyday. 🤤
.
🗣️Everyone was asking me if I was ready for the weekend and if I survived. I told them I feel good and it wasn’t bad. I was honestly was even surprised at myself to actually feel that good by the end of the week.
.
😇Positive thoughts, optimism, and putting life in perspective before a tough situation actually work!

⤵️Exercises for Low Back Pain

📊 Here's where the fun begins. The last few posts I talked about disc herniations and low back pain. And when people ask, what exercises should I do for my low back pain, it's impossible to answer without any knowledge of the patient.
-
Once people have moved from the high irritability stage to the moderate to low stage, there's a massive amount of variability. Some people I've worked with have no pain or radiating symptoms after the eval, some have intermittent pain, and a few can't seem to get rid of it. When attempting to select exercises and progressions for an individual, severity/irritability must go into the thought process.
-
Take home point: What exercise(s) would benefit you depend on a plethora of factors. If I just said try this random assortment of exercises without understanding you, your goals, and your symptoms, it would be of low value. And while there's nothing wrong with trying new exercises/movements, I'd be doing a disservice to you if I said everyone with low back pain NEEDS some specific fancy exercise. Use this information to establish a framework.

Directional Preference - Low Level

📊 In the last post in the series, I talked about how we don't treat the MRI, just the person in front of us.
-
🌐 That being said I want to talk a bit of an intro to directional preference. To overly simplify it, directional preference is a motion in any area of the body that with repeated loading (typically to end range) will improve ROM/pain/distal symptoms. Often times we can obtain this from a detailed subjective patient history by going into detail all of the things that make the symptoms worse, and all the things that make it better. We then attempt to classify a potential directional preference based on subjective history, and test it out during the objective to see if we can improve symptoms.

❌That still sounds a bit vague, so let's try an example. Steve comes in to see me because he has low back pain and symptoms into his right leg and foot. He tells me sitting, bending to tie his shoes, and driving aggravate his symptoms. He's better with supine or prone, and walking. I postulate that he may have a directional preference to extension, and attempt repeated movements to extension (swipe for video) to see if we can centralize his pain.

Again, overly simplistic here, but the term centralization refers to decreasing or getting rid of distal (further down the leg) pain. The goal is to make the symptoms in the leg more "central" compared to the distal symptoms, with the possibility that the more proximal symptoms may intensify. McKenzie Method is not just extension; Extension is typically more common.

🔑 Here's the take home message. Radiating and radicular pain can be debilitating. Go see a good physical therapist if your symptoms are worsening. The exercises I've posted are NOT in any way a comprehensive guide to directional preference. The exercises are opposite ends of the spectrum. The goal of either movement is to centralize symptoms (bring "up" the leg, not "down"). These are excellent starting points for low level exercise, more so in the highly irritable phase.

Let's Talk Disc Herniations

📊 Almost daily, I get a question from some one that says something along the lines of: "My MRI shows a disc hernation/buldge, what should I do for it?" Full disclosure, I love talking about back pain, and love viewing MRIs/imaging, however I treat humans. I do not treat your MRI.

🌐 If you swipe to the last picture, you'll see an image of a study done by Brinjikji looking at asymptomatic patients who obtained MRIs. The numbers show the % of people who had "pathological findings" on imaging, but those people had no pain. Take a look at the numbers by the decade. You can see how common it is as we age to have findings on imaging. We think of this as "Wrinkles on the inside." It's common knowledge that as we age our skin may get some wrinkles, but this is of little consequence outside of superficial beauty.

❌Does this mean that the x-ray/MRI/CT scan you have is useless, irrelevant, or worthless? Absolutely not. Diagnostic imaging simply reveals pathology, the history and the physical exam provide the relevance.

🔑 Here's the take home message. Pain is so much more than just an image. The clinical presentation of your symptoms is going to determine how and what I do when you work with me to treat your symptoms.

ATTN FELLOW SQUAT LOVERS: Do you ever look around in a work out and change your squat form because you notice your feet aren't turned out as much as the next person? Or that your stance needs to be wider because you saw someone else do it that way? Or maybe you are in a class being told how and where to position your feet but it just doesn't feel right to you?? ❓❓❓❓❓❓❓❓❓❓
Here's the deal: NOBODY HAS A SQUAT THAT LOOKS LIKE YOURS. Maybe your hip joints are a little more retroverted or anteverted, maybe you have more hip flexion range of motion at 45 degrees abduction rather than at 60 degrees. Maybe now your thinking "what do these words mean and how do I know if I'm retroverted or not?!" ‼️‼️‼️‼️‼️‼️‼️‼️‼️‼️
The point here is that your body will move in ways that are very different from the person next to you and there are many ways to cue your body into a happy medium between hips, feet, ankles, squat depth and proper range of motion. This nocebo example has two components:

1. Telling patients about their structural abnormalities and relating them to the treatment required can negatively impact their outcome.

There is no such thing as good or bad form, or posture. There is no right or wrong “exercise”. What does “functional exercise” even mean, and what is this “core” thing everyone keeps talking about whilst planking or crunching?! My newsfeed is blowing up with these so-called “fitness coaches” and “rehab experts” putting all type of crap out there. For 1 of 2 reasons, maybe both
1. Pure ignorance
2. Trying to grow their following by giving people what they think they want

The only bad posture is the posture we spend too much time in (shout-out Andreo Spina). Life is not perfect posture, and neither is sports
.
As human beings, our movement capacity should be vast. If we only stay in a small handfuls of postures we aren’t fulfilling our movement potential, and we’re creating more dysfunctions and weaknesses because of it. Your body will devolve rapidly
_
If you always hammer down the same posture, even if it’s “good posture”, you’re only creating strength and neurological control in that specific posture. So when your body slips out of that particular shape, weakness and injury is the outcome - and that’s not a good sign
_
There is no such thing as a functional exercise, just functional (or non-functional) joints that can fulfill movement potential. If your joints can’t successfully control a range of motion that is necessary in your daily life, you should probably make that a priority for your training practice
_
So in conclusion, is it that you need to practice good form (and never lose it, ever)...or, practice multiple forms.
_
How do we practice in those ways? Well, it’s going to take a philosophical change first and foremost. Understanding our biology and neurology is important
.
If you do not know how to train in such ways, find those who do.
_

This is Mayor Buch. Last year, he began having back pain. He had to travel two hours into Guatemala City to get PT. Knowing his community members couldn’t do the same he decided to bring PT to his community.
Last May, we collaborated with the town to start their first PT clinic. It’s now functioning with local students and PTs. Most importantly, the community members now have access to care.
Mayor Buch said, “We #ChoosePT.” We are so glad he did.

Why do we care about neutral spine and breathing with the shoulder?
⠀⠀⠀⠀⠀⠀⠀⠀⠀
In brief, when our ribcage is in a flared out position (shown early in video), our shoulder joints do not move efficiently. In order to fix this, our pelvis needs to be set, core needs to be active and we need to be able to breathe with our diaphragm. If we do not possess the ability to achieve this position well, our shoulder is subject to movement that is not optimal... we don’t want this!

See if you can keep your pelvis set, core tight, AND breathe, without losing overhead motion. ⠀⠀⠀⠀⠀⠀⠀⠀⠀
This is a great drill to work on overhead position with a neutral spine, hitting all points of performance (aka no cheating).
⠀⠀⠀⠀⠀⠀⠀⠀⠀
1. Set ribcage and pelvis with a forceful exhale, keep back flat on floor. ⠀⠀⠀⠀⠀⠀⠀⠀⠀
2. Raise your arm overhead as far as you can. ⠀⠀⠀⠀⠀⠀⠀⠀⠀
3. Take a slow and controlled diaphragmatic breath in, maintain that spine neutral / back flat on floor.
⠀⠀⠀⠀⠀⠀⠀⠀⠀
4. Keep that arm overhead for a few breaths, then return. Repeat for 5 breaths on each arm, 3-4 rounds.
⠀⠀⠀⠀⠀⠀⠀⠀⠀
...try repeating the arm overhead reach without setting the pelvis and see the difference. It is likely much easier to reach the ground. ⠀⠀⠀⠀⠀⠀⠀⠀⠀
Why is that? Most of the motion is likely coming from the lower back and rib flare... ever have back pain when performing an overhead lift 🤔

Tessa Waggoner, PT, MPT, CLT

Tessa was raised in Southern California and earned her Bachelor of Science Degree in Kinesiology from Cal State University, Northridge. In 1999, she went on to receive a Masters of Science in Physical Therapy from UC San Francisco and has enjoyed working in outpatient orthopaedic settings ever since. In 2001, Tessa received a Certificate of Completion for the Intensive Year Long Manual Therapy Course which was based on the Australian Approach to the management of Neuro-Orthopaedic patients.

Tessa also has a special interest in rehabilitating patients with cancer and cancer survivors who have edema. In 2004, she completed a special certification course with The Academy of Lymphatic Studies to become a Certified Lymphedema Therapist (CLT). She is currently enjoying helping patients with Lymphedema manage their swelling and improve their quality of life.

Tessa's favorite pastime is being with her young family. Spending time with her two daughters and keeping fit through running, yoga, pilates, and hiking helps to give balance to her life.

Never Stop Learning

At risk of sounding overly cliche, it is a never ending process.

When I first graduated from physical therapy school, it was easy to think that I knew it all. But there's a difference between knowing something and applying it in real world scenarios.

The more you learn, the more you realize what you don't know and that there's so much more out there to learn. (Cue Dunning-Krueger Graph)

🔑 Stress is an excellent way to grow, perhaps to level up one might even say (shout out @thelevelupInitiative). But the key is how you respond to the stressor. In that moment, I have no control over what time a patient shows up. Instead of complaining about things outside of my control, I chose to focus on the now and make the best of the situation. Shift your focus and energy on the shit you can make an impact on, and reap the benefits.

10/10 pain with every movement. Markedly poor posture. Can’t garden without pain.

Our PTs, working with local PTs and students, knew they could make a change.

Together, they screened her, took her through shoulder range of motion, and educated her on posture.

With a little hands on work, some simple soft tissue and joint mobilization, her affect began to change.

She left the clinic standing tall with less pain and a big smile on her face. What’s most important, she left clinic with a heart full of hope, excited to start gardening again.

New Running Impairments Post on Doctors of Running (link in description): Excess Anterior Knee Translation (Knee Forward to Toe). A common (possible) contributor to knee disorders during running. Just like a balanced squat form, your knees should stay behind your toes during midstance to decrease strain on the patellar femoral complex AND the Achilles’ tendon. The knee joint should bend to a max of 40-45 degrees on flat ground. Any more (ie 53 degrees in the photo), may indicate excessive loading of the quadriceps, possibly due calf or glute weakness. I worked with @27ashleybee to figure out a couple problems she was having. The first we identified was the above! Since she already has decent strength, we utilized a little cue to keep her knees back! “Lean with your nose.” This helped get her to lean from her hips, increased hip muscle use and decreasing load on her knees for her knee pain. Now she is getting ready for a killer half marathon. Good luck @27ashleybee !!

Is it muscle weakness or just muscle inhibition from trigger points? Check out these pre and post test results following dry needling of the hip abductors. 🍑 In the first video, the top leg is easily pushed down towards the table, indicating a possible weakness in the right hip muscles. 🍑 The second video is after one session of dry needling of the hip abductors. The left leg is able to withstand more force without dropping down. So was it a true muscle weakness? .
