Lumbar Degenerative Joint Disease/ Facet Syndrome:Common findings are limited side bending or extension, pain is provoked with posterior to anterior pressures on involved segments .
Sciatica/Disc Herniation: Typical findings include: (+) Lower limb tension test (+) Straight leg raise and (+)Slump test .
Lumbar Strain/Instabilities:Literature has suggested: (1) age < 40 yo, (2) avg (SLR) of 91° or greater, (3) the presence of aberrant lumbar movement, (4) + prone instability test make a stabilization program more likely to succeed .
Sacroiliac Joint Sprain: Posterior pelvic pain with onset commonly related to a fall onto buttock, misstep or abrupt weight bearing onto straightened leg. Also hx of pregnancy,
or child-bearing strain possible .
Spinal Stenosis: Affects those > 50 yo, Positions of extension commonly provocative
and flexion alleviating .
Psychogenic LBP, chronic pain or central sensitization:Tx pearls: Introduce pain as an emotional experience, stress sxs of pain not being = tissue damage, use graded exposure, decrease fear avoidance behavior, start cardiovascular program, improve functional movement .
