A 49-year-old man presented to the outpatient department with gradually progressive pedal edema and dull aching pain in the right hypochondrium. On examination, physicians found bilateral lower limb pitting pedal edema, mildly enlarged liver, a distended abdomen, and dilated and tortuous superficial veins on the patient’s abdominal wall (A), with a filling pattern from below upward. An echocardiogram showed a dilated inferior vena cava with substantial turbulence at the junction of the inferior vena cava and right atrium. An ascending venogram showing critical narrowing at the suprahepatic segment of the IVC with large collateral (*) (B) suggesting a membranous subtype of Budd–Chiari syndrome. Budd–Chiari syndrome is uncommon (estimated incidence of 0.87 per million population per year), characterized by either thrombotic or nonthrombotic occlusion of hepatic venous outflow. Systemic #Lupus #Cerebritis

Findings:
Scattered oval and flame shaped areas of abnormal signal are present in the #subcortical white matter bilaterally, involving the left #frontal lobe, bilateral #occipital lobes, and right #cerebellum. Abnormal signal is evident in the #brainstem as well.

Differential Diagnosis:
The appearance is relatively nonspecific and the differential is broad. Multifocal #ischemic foci are a possibility, due to #vasculitis or venous #thrombosis. This would be an atypical distribution for #MS or #ADEM. #Metastatic disease would not likely have this configuration of lesions.

Discussion:
Up to 7% of patients with #systemic lupus erythematosus develop a #CNS #vasculitis. #Pathologically, there is #perivascular #inflammation and #endothelial cell proliferation. Occipital involvement is characteristic, with sparing of the #periventricular regions. This helps to distinguish the process from #MS. Other manifestations of CNS lupus include atrophy, infarcts, and #hemorrhage. Infarcts in these patients may have several different etiologies, including #thrombotic (#hypercoagulable state), embolic (vegetations), or vasculitic. The vasculitic lesions occasionally respond to #steroids.

Sleep continues to fucking escape me so I decided to go for a brisk walk w/out my wardens (the kids). I know I can't quite run again just yet, which between that AND no #MuayThai it is just killing me. I feel like my muscles are dying w/out them. Seriously. Going from running 2-4 miles every other day and hitting the mats 3-5 times a week to absolutely NOTHING is just painful. Literally painful. My heart hurts and my body hurts just lying around. I mean, I totally understand what happened to my body and I know what caused it. STRESS. But the two things that were my stress relievers are what I've been missing.
So tonight I made an executive decision. I've been able to walk around a little more every day w/out being breathless or strained so I decided to kick it up a notch and push myself. Nothing too hard..just a good sweat to get the heart pumping again. Some sit-ups, push ups and stretching to warm up here at the house and a nice 1.5 mile brisk walk down through the neighborhood. I brought my phone and told Mr. Paul (my super awesome neighbor who just so happens to have hosed me down before so ya know, we're close...lol) where I was going...just in case.

blood in the #heart chambers
and leads to #clot #formation.
