Still one my most life altering rehabilitations. This is an extreme and interesting case because several factors contributed to this case of "gummy smile." 1: Maxillary excess
2: Extrusion
3: Severe periodontal disease
4: Class II jaw relationship

The prosthetic treatment was an immediate and implant retained overdentures. The most critical decision is deciding where to position the new central incisors. A concern were determining where the new lip position would be after surgery since the existing maxilla had too much lip support. When that is removed, the lip will relax and fall downwards but how much? How would you take impressions of the maxilla and take records? Every step was difficult but in the end, the results was life changing.

Hello everyone,
Today's post is about Bruxism.
I have talked about this before but if you missed my other posts I will re explain.
Bruxism is the dental term for teeth grinding.
This subject is important to me because I am a huge bruxer. This didn't start until recently, or maybe I am only aware of it now because I literally wake myself up in the middle of the night with the sound of tapping and grinding.
I don't have a partner, but usually a person realizes they grind when their partner tells them that they can't sleep because of the noise they are making, unless they have other symptoms.
The cause of bruxism can range from stress and anxiety to jaw problems and sleep apnea.
The symptoms can present as a dull constant headache, a sore jaw and worn down teeth.
The first image you see is of someone with a bruxing problem. The person has lost the definition on the bottom of their teeth and the cusps are flat. This is damaging to tooth structure. The enamel that is worn away causes sensitivity.
I will be seeing a physiotherapist to relieve the knot in my jaw as well as getting a proper night-guard made to ease the tension on my jaw.

This was a learning case. When I took this patients bridge off I found the abutments were unrestorable due to decay. The treatment plan was changed to extractions and implants with an implant bridge. Dentistry can throw you curve balls and you always have to be on your toes. Happy Tuesday everyone!
When you go to the dentist does your hygienist use an intra oral camera?
I took this picture of my mouth today using just my cell phone, but I think that it is extremely important for dental offices to have an intra oral camera.
The benefit of having a camera that fits inside the mouth is for the patient to be able to view the problem areas and things that they need to work on. This is also a great way to keep a record of what the mouth looks like at each visit. I highlighted two areas that I could point out to a patient.
At the top you will notice that I have a burn on my palate. I am actually in quite a lot of pain from it. As dental professionals, it is important that we inquire about anything abnormal.
In the back I circled the small red circular vessel. I personally don't think there is anything there but this is an example of how small a cancerous lesion could be in its early stages. This could be something that we can refer back to you if it is a bigger next time.
Another nice idea is to take a picture before and after a cleaning. Patients enjoy seeing results and hygienists can explain their oral hygiene instructions using the pictures.

Whitening.
Everyone wants to whiten their teeth. This has become one of the most requested treatments in the dental clinic in recent years.
Did you know that not everyone is eligible for whitening treatment?
A responsible dental clinic will turn you away for whitening treatment if you're mouth is not in a healthy condition. For example, if you haven't had a cleaning in years and your teeth are covered in calculus, you shouldn't have whitening treatment done. Prior to whitening it is best to remove all of the plaque and biofilm from the services of your teeth for maximum results. Also, if you are pregnant it is not recommended that you get a whitening treatment done. Obviously there are chemicals in whitening solution that could potentially be harmful if ingested.
People that are suffering from periodontal diseases such as gingivitis and periodontitis should not receive whitening treatment until their condition has improved. This is an incentive for people to work on their oral hygiene routine at home.
Be wary of whitening treatment that you see in a kiosk in the mall. First of all, these people are most likely not dentists or hygienists therefore they do not have the proper education when it comes to whitening treatment. Although they may have better deals than a dental office, things could go wrong. I have heard stories of people with severe periodontal disease receiving whitening treatment from these type of places. No responsible dental professional would allow this, therefore I strongly recommend passing by these places.

Very interesting case. Tooth 7,8,9 had severe periodontal problems and needed to the removed. Pt wanted a new smile and due to finances and bone quality we decided on a fixed bridge. Tooth 6,7,11 were prepared and a six unit bridge was fabricated. The soft tissue healed so nicely and papilla are present. When the patient comes back for a follow up in a few days I will be sure to get a picture of the gums. β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€”β€” DM us to be featured!πŸ“₯

Veneers Macro Photograpy.
Natural looking veneers are possible only if we are able to recreate features as shade, shape and morphology.
Teeth are never just yellow or white. There's always transition from yellowish dentin near cervical part to more light and transparent enamel at the incisal edge.
Morphology, or surface isn't flat, that's why different parts of a tooth reflect light in a different way giving it a unique 3D look.

This is an extreme example of gingival recession or gum recession.
Recession can be caused by various factors but when it occurs it can be painful because the root of the tooth is exposed.
Brushing with a medium or hard bristled toothbrush can be damaging to the gums, causing them to recede. To prevent this, ensure that your toothbrush is soft bristled and that when you brush you aren't using pressure. Biofilm is soft and removed easily when done regularly. Don't overdo it.
Teeth have a mind of their own and grow in all different directions. If the tooth is rotated or overlapping this can be an area where recession occurs. Also, occasionally during orthodontic treatment recession can occur from the teeth being moved around.
Lack of proper oral hygiene causes gingivitis and periodontitis. The ligaments which hold your teeth in place are destroyed by the disease process and the gums begin to recede and can look like this picture. The teeth are being held in place and are most likely mobile. This is no way to live. Take care of your teeth and gums now if you still have them.
If you do suffer from recession from genetic reasons or from previous poor oral hygiene habits, you can see a periodontist that can perform gum surgery. This can help with the problem.

Did you know that bone volume under fixed bridges reduces over time. Natural teeth or dental implants provide stress and stimulus to bone tissue and therefore cause the bone to be maintained. If we replace missing teeth with fixed bridges it is quite common to see bone under these fixed bridges to become both soft and thin/reduced in height over time. In this case we had to use zygomatic implants to replace the upper arch.
